State Releases Report On Boy's Starvation Death

State officials have released their review of the starvation death of a 7-year-old boy and found miscommunication between agencies let his case fall through the cracks.

Chandler Ashton Grafner weighed just 31 pounds when he died in May. A court document said he was kept in a closet for days at a time without food, water or access to a bathroom.

The boy's guardian, Jon Phillips, 26, and his girlfriend, Sarah Berry, 22 face charges of first-degree murder and child abuse resulting in death.

During a preliminary hearing for the two earlier, homicide Detective Larry Moore said Chandler "looked like a concentration camp prisoner. He was so thin, there was no body fat ... His eyes were sunken. His cheeks were sunken."

He told the court that the closet that the boy was locked in was covered in feces and smelled of urine.

Their lawyers have said argued that the boy had a number of pre-existing medical conditions that could led to his death.

The couple is set to stand trial on Sept. 24.

Grafner's death raised questions about how well the social service system responded in the case.

Teachers at Grafner's school have said they reported concerns about the boy’s frequent absences and other possible signs of abuse.

At least three agencies were responsible for investigating the welfare of Chandler, but the state report released Tuesday indicated they didn't do an adequate job or share information. The investigation confirms an earlier report by 7NEWS Investigators.

Specifically, the report found the Arapahoe County Department of Human Services staff did not follow-up on the whereabouts of Christina Grafner and her children when they found out the address listed for her was not valid.

The report also found a need for Jefferson County's Department of Social Service to to a more comprehensive assessment of John Phillips and Sarah Berry after they did not follow through with appointments.

According to the state, the Jefferson County Department of Human Services didn't check reports from schools that Chandler had bruises. The state also says the county failed to search for his parents and didn't force his guardian to provide for his mental and developmental needs.

The state says the Denver Department of Human Services failed to get reports from Jefferson County after it was told of alleged abuses in January.

Tuesday's report found several county agencies not complying with state regulations, including Jefferson County's failure to update the statewide computer system for child welfare, called TRAILS.
